At The Mpowerment Alliance, our mission is to provide expert forensic psychological services and comprehensive mental health disability evaluations with professionalism, integrity, and compassion. We are dedicated to delivering objective, evidence-based assessments to support legal processes, disability determinations, and the overall well-being of individuals and communities.
You have been scheduled for a mental health evaluation at the request of the Social Security Disability Adjudication Services (DAS). The purpose of the appointment is to understand your symptoms and how they affect your daily life. This appointment is not a therapy session, and no treatment or referrals will be provided. For more clarification about the purpose of your appointment, please contact your DAS adjudicator.
All scheduling concerns must be addressed to the Social Security Disability Office. Everyone has a different disability adjudicator, so be sure to look at your paperwork for their direct number. If you cannot find your representative's number, no worries, call (678) 639-2100 for the main line.
You should expect to be with us anywhere between 1-3 hours, depending on the services requested by Disability Adjudication Services (DAS).
We offer a 15-minute grace period for all scheduled appointments. If you arrive more than 15 minutes late, your appointment will need to be rescheduled through Social Security. If you anticipate being more than 15 minutes late, please do not continue to the appointment, as we will be unable to see you.
If you are not feeling well or exhibiting any signs of illness on the day of your appointment, please contact Social Security Disability to reschedule your appointment. Be advised, you may still be asked to reschedule if you appear ill, even if symptoms were not disclosed.
A written psychological report will be prepared and submitted to Disability Adjudication Services (DAS) within 10 days of your appointment.
Our office does not make disability determinations, approve or deny claims, or control the outcome of your case. All decisions are made by DAS based on a review of multiple sources of information.
If you would like to request a copy of your evaluation report, please contact DAS directly, as they manage all record requests and will guide you through the appropriate process.
If you are eligible for a travel reimbursement, please sign and return your travel voucher directly to DAS, who will process the reimbursement. Any additional information or documentation you believe is relevant to your case should also be submitted directly to DAS for inclusion in your disability file.
